Appointments
Appointments can be made with us by telephone or in person at our office. If for any reason the physician you requested is not available, we will try to accommodate you or get you scheduled with one of the other providers. Please arrive 15 minutes before your scheduled appointment to complete any missing paperwork.
Your appointment will be confirmed 3 days before by telephone. Should you need to reschedule or cancel your appointment with us, we ask that you do so by phone 48 hours in advance.
ADVANCED PRACTICE PROVIDERS (APP) (PDF)
The American Urological Association (AUA) endorses the use of APPs in the care of genitourinary disease through a formally defined, supervisory role with a board-certified urologist, under the auspices of applicable state law. There may be times where you will be treated by one of our Advanced Practice Providers rather than of one of the Urologists – Dr. Cunningham, Dr. Chen, Dr. Nixon or Dr. Montgomery.
